Verify that your employer has registered with the program. If not, follow the steps above, or call the Hotline (510) 433-0320. We would be happy to contact your employer about registering for the program.

Each employee must complete an employee registration form and liability waiver. You can download the employee registration form here in PDF format or go to our on-line registration form (note: you will need to download, print and mail in the liability waiver separately). You may also call the Hotline (510) 433-0320 to request a form or obtain one from your employer contact person.
